- Birth Certificate
- Physical
- FL Certificate of Immunization
- Social Security
- Proof of Residency (one of the following)
- Warranty Deed or Homestead Exemption
- Lease and a recent utility bill or initial order for the service PLUS one of the following items below with correct name and address
- Driver license
- Auto registration
- Voter's registration
If applicable, legal documents i.e., a copy of any current judgment of divorce(dissolution of marriage) or other court order establishing the right of custody will be required for registration
New Online Process for Returning Student Registration through Family Access
Family Information updates and the "1st Day Packet" required of all returning students may be completed online through Family Access for the school year. Family Access will open for parents/guardians to review and update family information (address, phone#, emergency contact, etc.) and complete the "1st Day Packet" (Emergency Contact Card, School Security Form, Release form for Field Trips and more). Please note that a new proof of residency is required by the school for all address changes.
